






Many of us dream of a bright, confident smile, but the thought of dental surgery—especially a cosmetic procedure—can be daunting. One common aesthetic concern that leads people to consider surgery is a "gummy smile," where an excessive amount of gum tissue is visible when smiling.
Fortunately, modern dentistry offers solutions. A recent study by Prof. Altayeb's team provides powerful new insights by comparing two advanced laser techniques for correcting this issue, both part of a modern push towards minimally invasive dentistry designed for greater precision and patient comfort. The first is a traditional "open-flap" (OF) method, which involves creating and repositioning a flap of gum tissue. The second is a minimally invasive "flapless" (FL) method. While both techniques create beautiful results, the study revealed surprising and impactful differences, not in the final look, but in the patient's journey to get there.

Both open-flap and flapless methods achieve successful, predictable esthetic outcomes for treating "gummy smile." After 9 months, no significant difference existed in gingival tissue rebound between groups.
Flapless patients reported significantly lower post-operative morbidity, including reduced anxiety, pain, swelling, bleeding, and tooth sensitivity, requiring less pain medication.

The study employed a rigorous randomized controlled trial design with 36 healthy patients aged 22-45 with altered passive eruption requiring crown lengthening on six maxillary anterior teeth. Participants were randomly allocated into two equal groups of 18 patients each.
All surgical procedures were performed by a single operator using an Er,Cr:YSGG laser for precision ablation of both soft and hard tissues, ensuring consistency and eliminating operator variability in the results.

Traditional approach involving apically positioned flap creation, followed by osseous recontouring. The flap is then repositioned and sutured, requiring more extensive tissue manipulation and healing time.

Minimally invasive approach with initial gingivectomy followed by osseous recontouring performed through the gingival sulcus without raising a flap, resulting in less tissue trauma and faster healing.


The research established that both laser-assisted techniques are predictable and effective, producing comparable and stable clinical results after 9 months of follow-up. The gingival margin level showed initial coronal growth in the first 3 months but demonstrated significant stability between 3-month and 9-month follow-ups for both groups.

A critical finding revealed that tissue rebound was significantly higher in patients with thick periodontal phenotype compared to thin phenotype, regardless of surgical method. This highlights the importance of phenotype assessment in treatment planning. The OF group showed slightly higher Gingival Index and Bleeding on Probing scores at 1-month follow-up due to greater tissue trauma, but these differences resolved by 3 months.



While clinically equivalent, the two techniques produced vastly different patient experiences. The flapless procedure was associated with significantly lower post-operative morbidity and more positive overall perception across all measured parameters.
The difference in patient perception of surgical difficulty was particularly striking. In the flapless group, 56% found the surgery less challenging than anticipated, while only 5% of open-flap patients reported the same experience.

The time savings, combined with reduced post-operative complications, creates a more positive experience for both patients and practitioners.

The unanimous positive response from flapless patients underscores the superior patient experience achieved through this minimally invasive approach.
While still positive, the open-flap results show notably lower patient willingness to repeat the experience, reflecting the increased morbidity associated with this technique.
